The State of Indiana requires a Cremation Authorization Form to be signed by the family. This form is typically signed at the funeral home, and is only listed here for reference purposes only.

 

If a Veteran as passed away, he or she is eligible for benefits from the county. The family should take these steps to file for the county benefit:

–Print out Form 49890 (On Account of Appropriation for Burial of Veterans) and complete

–Mail that form, and send to the county Veterans Administration Office, along with the following:

One Certified Death Certificate

A photocopy of the Veteran’s DD214 (Veteran discharge papers)

Discharge Documentation (Form DD-214) is required to file for Veteran’s benefits. If you cannot locate the Veteran’s DD-214 Form and need to acquire another copy, request one online (Standard Form 180) with the National Archives.

A family of a Veteran can request a United States flag. The family should print and complete the Application for United States Flag and bring to Geisen Funeral Home’s Crown Point location. The funeral home staff will obtain the U.S. flag, and the family can pick up the flag at the funeral home when it’s ready.

A family of a Veteran can request a Governor’s Memorial Certificate. The family should print and complete the Request for Governor’s Memorial Certificate – then follow instructions on the form to mail or fax that form, along with a photocopy of the Veteran’s DD214 (discharge papers).

A family of a Veteran can request a Presidential Memorial Certificate. The family should print and complete the Presidential Memorial Certificate Request Form – then follow instruction on the form to mail or fax that form, along with a photocopy of the Veteran’s DD214 (discharge papers).
